There are three requirements for your child to be accepted within Pinebelt Head Start,\u00a0Inc. Now you can pre-enroll your child online, just click on PineBelt Forms<\/p>\n<p>\u200bIf you prefer you can Download the pre-enrollment form, over the phone or you can visit our two centers, or our Main Office\u00a0Documents needed:<\/p>\n<ul>\n<li>Proof of age<\/li>\n<li>Proof of income<\/li>\n<li>Current Physical with updated Immunization records<\/li>\n<li>Health Insurance Card<\/li>\n<li>Dental visit results<\/li>\n<li>Health treatment plan with any listed allergies included<\/li>\n<\/ul>\n<p>\u200b\u200bYou will be contacted within 5 business days by a Family Service Advocate (FSA) to set up the enrollment\u00a0appointment.
